Please click on the buttons below to access the camp waiver forms. Parents are required to fill out these forms in order for their child to attend camp. Once completed, you can either print and email the forms back to amyp@visaliafirst.com or print and bring them to the office during office hours (Monday - Friday, 8:30 AM - 4:30 PM).
